To Enroll Your Child

1. Contact the center nearest to you.

2. Meet with the Family Advocate or Family Services Supervisor to apply. (We do the paperwork.)

You will need your child’s birth certificate, insurance information, immunization records, and verification of income (such as a recent tax return or paycheck stub). The child’s Social Security number is helpful if available.

3. Once your application is completed and processed, you will receive a letter stating whether or not your child has been accepted or placed on our waiting list.

4. If accepted, you will also receive a “Getting to Know You” home visit by the teacher to prepare your child to start classes.

  • Current Immunization Record on pre-school form/
Las inmunizaciones actuales escritas en el formulario para guarderías (pre-escolar).
(get this from your doctor’s office or the health department/Pídalo en la oficina de su médico o el Departamento de Salud)

  • Physical Form completed with Lead Level, Hemoglobin/Hematocrit
El formulario del examen físico con el nivel de plomo, hemoglobina y hematocrito
(Head Start provides the form/En el formulario de Head Start)

  • Dentist Form completed
El formulario para el Dentista completado.

  • Proof of Income
Una prueba de ingreso
(such as a copy of last year’s income tax return or most recent paycheck stub/como una copia de los formularios de los impuestos del año pasado o un talonario de cheque reciente)

  • Certified Birth Certificate for 3 and 4 year olds
El Acta de Nacimiento certificado para niños de 3 a 4 años de edad.
(can use Mother’s Copy for infants and toddlers but not for 3 and 4 year olds) (Puede usar la copia de la madre para bebés y niños pequeños pero no para niños de 3 a 4 años de edad.)

  • Social Security Card

  • Insurance or TennCare information

  • Proof of Living Residence
Una prueba de residencia
(such as an electric bill, phone bill, water bill, rent or mortgage statement or lease agreement/factura de luz, teléfono, agua, el alquiler o la hipoteca, etc.)

  • Copy of any pre-existing plans with other agencies
Una copia de cualquier otro plan preexistente de otra
(example: The Personal Responsibility Plans from the Families First Program/ por ejemplo, planes de responsabilidad personal del Programa de Families First)

  • Copy of any Custody Papers that may exist for the child
